You must complete the self-reporting form for each rapid test performed, regardless of whether the result is positive or negative. Invalid results should not be reported.

Once in the platform, you will be prompted to provide some identifying information, including:

• First and last name
• Date of birth
• Health insurance number

To confirm your identity and protect your personal information, you will also be asked to select the email address or mobile phone number you provided at the time of vaccination to receive the download link.
